COMMUNITY SERVICES BLOCK GRANT (CSBG)
The Community Services Block Grant (CSBG) assures that funds are made available to support activities designed to assist low-income families and individuals. The grant helps to remove obstacles and helps to solve problems that block the achievement of self-sufficiency for families and individuals seeking to secure and retain meaningful employment or an adequate education. Also, Health, Nutrition, and Safety components afford the opportunity to provide health and hygiene supplies, meals and other health safety equipment such as fans and air conditioners for senior citizens and other low-income eligible people throughout our service communities.
LOW INCOME HOME ENERGY ASSISTANCE PROGRAM (LIHEAP)
The Low-Income Home Energy Assistance Program (LIHEAP) Grant provides appropriate and timely assistance to eligible households to pay home energy bills and to assist with other energy related services. Households that consume energy for residential purposes and meet the requirements of the program will be eligible for assistance. Emphasis is placed on the elderly and disabled.

To place your name on the waiting list to receive an appointment go to http://shcp.itfrontdesk.com and follow the instructions. When your appointment has been scheduled you will receive a text message and a call from the agency confirming the date, time, and documentation you will need to bring to the appointment. - Picture ID for household members eighteen (18) years or older
- U.S. Passport
- Driver’s License
- State ID
- Employment ID
- School, College/University ID, Personal ID
- U.S. Military ID
- Tribal ID, Alien Registration/Permanent Registration Card
- Temporary Residence Card
- Social Security Card(s)
- Certified copies of Social Security Cards for all household members.
- Affidavit (notarized)

- Proof of household Income
- Award letter(s) form (Social Security, SSI, Disability, etc.)
- Unemployment determination letter
- Other documents to verify income (Check stubs) last 30 days
- If household receives no income, a zero Affidavit(notarized).
- Contribution letter
- Residence Verification
- Copy/copied of utility bills in the household
- Lease Agreement
- Mortgage document(s) deed, etc.
- Marital status proof(affidavit)
